Documentation needed:
– Original and photocopy of the DNI or NIE and passport of the legal guardians.
– Family book or birth certificate in the case of not having a family book.
– Collective registration certificate (padron)
– Work activity:
a) Employed work activity:
– Complete and certified work life report
– The personnel belonging to MUFACE, ISFAS or MUGEJU will present a certificate from the work center indicating the hours of weekly dedication.
b) Self-employment activity:
– Complete work life report and certificate that proves to be registered in the Economic Activities Tax (last self-employed payment stamp, corresponding to the month of March)
– Statement of responsibility indicating the hours of weekly dedication.
People who work outside of Spain and/or seasonal workers on agricultural holdings must also provide supporting documentation that they are carrying out a work activity at the end of the application submission period.
– Vaccination record and social security card or private medical insurance instead.
